On page 42 of the user manual, it mentions that is possible to create a virtual four.channel device with Jack.

I've tried this but I cannot get it to work, I can only hear the two channels coming from Quicktime, but I cannot hear the microphone channel.

How do I configure this with the Record to new file option where it only shows two channels and not three (or four)?, even though I can see in Jack Connections manager that Amadeus is listed as having an in3 channel... which I even tried to connect the output from Garageband (which is picking the mike input) to this in3 channel to no avail.

Please advice as to how this task can be accomplished
